It's not uncommon to a pitcher to throw a wild pitch once in a while. However, it is unusual for a pitcher to throw a wild pitch during an intentional walk.
It happened in the bottom of the third inning at Comerica Park in Detroit yesterday. The Tigers' Miguel Cabrera was on second base with two outs, and Kansas City Royals pitcher Danny Duffy was in the middle of an intentional walk to batter Victor Martinez. However, the 3-0 pitch from Duffy was lobbed way over catcher Brayan Pena's head and into the backstop, which gave Miguel Cabrera a chance to advance to third base.
So according to the official score, it still counted as an intentional walk to Martinez, but also a wild pitch for Duffy. Fortunately, Duffy was able to strike out the next Tigers batter, so no runs scored as a result.
